The Community Integrated Model is designed by Imago Dei Consulting Services, LLC to connect individuals and families to strategic resources in their neighborhoods.

Individuals and families that are part of Winner’s Affordable Housing Program, LLC will have a list of partnership referrals to various programs and resources that will help them thrive. They will feel a sense of belonging and stay connected in the community. A community care manager will meet with each person or family group to create an individualized plan of care that will identify priority needs and make adjustments that suits client’s preferences. In addition to this, the community care manager will host a once a month workshop at the client’s residence that will focus on important life skills, because we believe that everyone has the capacity to be successful and thrive!
